The Eisai USA Foundation, Inc.
As our charitable arm, The Eisai USA Foundation, Inc. provides support and funding to U.S organizations that offer initiatives for patients and caregivers; champion innovation in medical and pharmaceutical sciences; offer disaster relief efforts; and fund community activities in the areas where our employees live and work.
Our Mission
We are a human health care (hhc) company. Our mission is to give first thought to patients and their families, and to increasing the benefits that health care provides.
The Eisai USA Foundation provides charitable donations to support:
- Health-related initiatives for patients and caregivers, including those that address disparities in healthcare and social determinants of health (i.e., food insecurities, transportation needs, culturally relevant and accessible education, etc.);
- Innovations in medical/pharmaceutical science that address the stigma in both cancer and AD/dementia, raise awareness, and/or provide information on brain health
- Patient navigation and support services in institution and community settings
- Disaster relief effort
- Community activities in the areas in which Eisai Inc.’s employees live and work.
Who is Eligible
Organizations submitting a request must be either:
- A charitable organization exempt from income tax under section 501(c)(3) of the Internal Revenue Code that is not classified as a private foundation under section 509(a)
- A governmental unit within the meaning of section 170(c)(1) of the Internal Revenue Code
How to Apply
All donation requests must include these supporting materials:
- The proposed use/specific objective of the requested funds, including any donation proposal submitted by the potential donation recipient
- The mission, background, affiliation and the history of the organization
- A Federal Tax Letter that proves their 501(c)(3) status
- The financial statement(s) of the organization, including any facts that may raise conflict of interest or self-dealing concerns
- A current version of the organization's W-9 form
