At Eisai Inc. (together with our subsidiaries collectively known as the “Company”), we are committed to ensuring the privacy of the personal information we collect, use and disclose about the patients, healthcare providers, employees and other business colleagues with whom we interact.

If you would like to make a request to understand what personal information we have on you and/or delete such information, you must fill out the information below or call us toll-free at 1-833-746-2910. (Please make sure to upgrade your browser to Google Chrome, Mozilla Firefox, Apple Safari or Microsoft Edge before filling out the form below.) Before we can honor your request, we are required by law to verify your identity. In other words, we have to make sure you are who you say you are. We may need to follow up with you to get any additional information necessary to verify your identity and/or respond to your request. If you have authorized a third party to make a request on your behalf, please note that we will need written documentation of such authorization and that additional verification may be required. Please keep in mind that, when you make a request on behalf of another person, it may take longer to process the request because the Company is required by law to confirm you are properly authorized to make the request.

We reserve the right to reject, in whole or in part, information requests under various circumstances, including, but not limited to:

  • We cannot verify the identity of the person making the request
  • The requestor is an employee, consultant or otherwise has a business-to-business relationship with the Company
  • The information is exempt from disclosure or deletion under the applicable law (e.g., clinical trial data).
